1.  Proof of Business

1.      Tax Id number or seller permit- If you have a US based company, your are issued a Federal Tax ID number by your state. We need this to verify your business

2.      Company Name- Please state your business name accordingly based on your business cards, flyers etc.

3.      A Valid email address- List an email address that you check daily.

4.      A Valid phone number- List a proper phone number where we may contact you, if need be regarding your order.
